Position Summary: CommUNITY Support Team is a Community Mobilization initiative that brings individuals, families, and their supporters together to work towards a better life. Support provided to youth, individuals and families is grounded in the core values and practice principles of High Fidelity Wrap Around.

The Mental Health Facilitator will support CommUNITY participants who require mental health services or have identified them as a goal through group facilitation, community outreach, and system navigation. This role focuses on resource coordination and mental wellness support and education. This position is held by the Community Exchange and will work in collaboration with the CommUNITY Coordinator & CommUNITY Mental Health Facilitator.
